First time visitors

Prior to your first appointment, a new patient packet will be mailed to you, if time allows. Please fill it out as completely as possible. If you do not receive the packet, we ask that you come 30 minutes early to your appointment and fill out the paperwork in our office. Another option is to print the paperwork from our website, and bring it with you.

When you arrive, our front desk staff will check you in. If the new patient packet was completed and returned through the mail, they will verify any missing information. If you brought the new patient packet to your appointment, they will take and process it.

You will need to present your insurance cards and prescription card (if applicable). We will also ask to take your picture for your chart.

Please bring information regarding your cancer policy or other supplemental insurance (if applicable). If you have an advance directive, such as a living will or power of attorney document, you may bring it and we will put it in your chart.

During your first consult as a new patient, our physicians will review in detail your past and current medical issues and concerns. Your diagnosis will be explained, treatment options discussed, and questions answered. A cancer or blood diagnosis can be scary and we want each patient to know we are here for them from start to finish.
